The purpose of this form is to collect feedback from users about the NSW Pet Registry Litter Log.


This information will help the Pet Registry team understand users’ experience and inform ongoing development and improvements to the NSW Pet Registry.


Litter Log information will not be updated or changed by completing this form.

The Office of Local Government, as part of the Department of Planning, Housing and Infrastructure, is collecting your personal information for the purpose of improving the Litter Log system, investigating any particular issues specific to your account and to contact you if you have requested contact.


Providing this information is voluntary, however, if you choose not to provide it, we may not be able to investigate specific issues raised in the survey or contact you with any follow up questions.


Your personal information will be held by the Office of Local Government and handled in accordance with the Privacy and Personal Information Protection Act 1998 (NSW).


Your information will not be disclosed to any third party unless required by law. You may request access to, or correction of, your personal information by contacting privacy@dpie.nsw.gov.au
