This form contains information that identifies you and is defined as ‘personal information’ under the Privacy and Personal Information Act 1998 (NSW) (PPIP Act). Supply of this information is voluntary, but you may not be able to be registered under the Code of Practice if the information is not provided.

It is an offence to provide false or misleading information in applications and documents in connection with matters under the Biodiversity Conservation Act 2016.

The Department of Climate Change, Energy, the Environment and Water (the department) collects and manages the personal information you provide in accordance with the PPIP Act. Information supplied on this form will be used by officers of the department to process your registration under the Code. Your personal information may be disclosed to third parties for the purposes of confirming information provided by you, and for law enforcement purposes. The department will not otherwise use your personal information except for purposes listed as exemptions in the PPIP Act.

When you purchase, trade or export with a licence holder or another person registered under the Code of Practice they are required to record and store details of those transactions, including some of your personal information. This information is provided to the department for information and compliance purposes, including randomly checking compliance. When you buy or sell to someone you must tell them that you are recording their information and that it may be provided to the department.

Specimen log

Each individual animal and their associated specimen information must be registered. To register multiple specimens sourced from an individual animal or specimens sourced from multiple animals, please repeat the process until you have registered all the protected animal specimens you possess or intend to collect.

Note:
If you have specimens from the same species of animal but they are from different individuals (e.g. bones from two different wombats), you will need to log each animal's specimens separately.

If you have specimens from multiple animals of the same species, log each individual animal separately
If you are not able to find the species of the specimen in this list, please select 'Other' at the bottom of the list. You will be asked to provide the species common name and scientific name.





Transactions
There is an enforceable limit of 5 transactions that you can make in a 12-month period.

By selecting 'Bought' this will be automatically included as part of your transaction record for this 12-month period.


If you buy and sell any additional protected animal specimens you are required to report this information annually by your reporting date (12 month anniversary of your registration to the code).



If this land is owned by someone else, please retain the written consent in your own records of the private landholder giving you permission to take the specimen.

List all the specimens from this individual animal


If you have multiple specimens of the same type from the same species you have selected (e.g. 3 non-viable eggs of a tawny frogmouth or 2 taxidermied common wombats), you are required to log each specimen as a separate individual animal.
Unpreserved specimen
You must take this carcass or unpreserved specimen to someone legally authorised to preserve it (such as a BC licensed taxidemist) within three months of first possessing it.

E.g. If you have 3 feathers from a bird, you would enter '3' in this field.
